PMDD Appointment Preparation Sheet & Clinician Conversation Guide by The Luteal Edit

The PMDD Appointment Preparation Sheet & Clinician Conversation Guide is a structured, practical handover tool designed to bridge the gap between your daily lived experience and your healthcare appointment. It condenses multi-cycle tracking data, symptom profile, functional impact, safety assessment and treatment history into a concise format that helps your clinician understand your cyclical pattern clearly.

Please note: This tool is designed to help you organise information for a healthcare appointment. It does not diagnose PMDD or replace individual medical advice.

How to use it (alongside your tracker)

Step 1: Complete the preparation sheet after tracking

  • Fill out this sheet a day or two before your appointment, drawing directly from your completed daily logs, cycle tracking apps or paper diaries.

  • Fill in your cycle summary including last period, cycle length and the timing of your symptom severity.

Step 2: Map your top symptoms and functional impact

  • Tick your main symptoms and mark your top 3 most disruptive ones.

  • Rate how severely your functioning is impacted on your worst days across areas like work, relationships and self-care.

Step 3: Document safety and history accurately

  • Complete the safety check honestly to note any psychological distress or ideation experienced particularly during your symptomatic window.

  • Fill out past interventions you have tried, noting what helped and what caused side effects so you do not have to recall them on the spot.

Step 4: Bring it to your appointment

  • Bring this printable guide alongside your multi-cycle tracking logs to your appointment.

  • Let the conversation guide prompt you on how to explain the timing, severity and the pattern of your symptoms and consider the appropriate requests for your clinician.
